The Service

The Portsmouth City Rehabilitation and Reablement Team provides assessment, time limited intervention and support to adults referred following a decline in independence and/or a significant increase in their care needs as the result of acute illness, or the worsening of one or more long term conditions, and/or may require rehabilitation/reablement to improve their functional ability to improve independence.

The team reaches into QAH to support the transfer of care of patients medically fit for discharge following an acute in patient episode of care.

The team has alignment with the Community ED Team and the Hospital Discharge Team

What is the role? • To assist service users to meet their social and physical rehabilitation goals working to individual care plans. • To enable service users to achieve skills for independent living i.e. household tasks, shopping, cooking, interpersonal skills and personal care, which may involve moving and handling. • Escort service users as required on appointments to hospital, doctors, home visits etc. To provide after care and support in accordance with the outcome of the service user's appointment. • To report promptly and discuss significant changes in service users to appropriate professionals. • To maintain confidential, legible and accurate records. • Work jointly and effectively with others including internal staff and external health care providers. • To attend regular supervision sessions and participate in monthly staff meetings.

Who is the person?

You need to:

• Have the required level of literacy and numeracy to enable you to accurately report and record service user records. • Have an understanding of the needs of the client group, promoting and supporting individuals' rights and choices. • Have the ability to be patient and to sensitively assist service users to maintain their independence. • Have excellent communication skills. • Be able to maintain confidentiality. • Be able to offer social and emotional support. • Be flexible and able to adjust to the ever changing client needs. • Be able to work as an effective team member of the service. • Work flexibly between the hours of 7am and 10pm in order to meet the needs of the service. This includes working alternate weekends. • Ideally you will have a full driving licence and your own transport as your role will involve visiting service users across the city on a daily basis within a specific time span.
