Salary: £15,953 - £17,077 pro rata plus a 17% enhancement for working unsocial hours. 37 hours per week, part time hours will be considered - which will include early, late and weekend shifts.

This is an exciting opportunity to work in a new service. The Front End Reablement Service based in Adult Social Care will deliver reablement and rehabilitation interventions to Portsmouth residents in their own homes. The post holder will provide skilled support through targeted rehabilitation and reablement programmes in order to restore, maximise or prevent deterioration in service users physical, psychological and social functioning. We will provide comprehensive training to support you in obtaining the Care Certificate.

You will need to be able to work flexibly between the hours of 7am and 10pm in order to meet the needs of the service. This includes working alternate weekends. Also ideally you will have a full driving licence and your own transport as your role will involve visiting service users across the city on a daily basis within a specific time span.
