Salary: 16,203 - 17,270 pro rata plus a 17% enhancement for working unsocial hours.

37 hours per week, part time hours will be considered - which will include early, late and weekend shifts.

The Service

This is an exciting opportunity to work in a new service. The Community Independence Service based in Adult Social Care will deliver reablement and rehabilitation interventions to Portsmouth residents in their own homes. The post holder will provide skilled support through targeted rehabilitation and reablement programmes in order to restore, maximise or prevent deterioration in service users physical, psychological and social functioning. We will provide comprehensive training to support you in obtaining the Care Certificate.

What is the role?

To work at all times to a rehabilitation philosophy enabling clients to be as independent as possible. To deliver structured rehabilitation and reablement programmes to clients in their own homes. To assist clients in a range of daily living tasks, for example with transfers, mobility, washing and dressing, toileting, household tasks, meal preparation, social activity and access to the community. To have an understanding of and be able to implement safe moving and handling techniques, promoting health and safety for service users and their carers. To report promptly and discuss significant changes in service users abilities to appropriate professionals. To maintain confidential, legible and accurate records. Work jointly and effectively with others including internal and external staff. To attend regular supervision sessions You will receive appropriate training and will achieve competencies leading to the Care Certificate.

Who is the person?

You need to:

Have an understanding of the needs of the client group, promoting and supporting individuals' rights and choices. Have the ability to be patient and to sensitively assist service users to maintain their independence. Have the required level of literacy and numeracy to enable you to accurately report and record. Listen to others and communicate with sensitivity and understanding Act with integrity, honesty and impartiality. Be able to maintain confidentiality. Be able to offer social and emotional support to clients and their carers. Be flexible and able to adjust to the ever changing client needs. Be able to work as an effective team member of the service. Have a positive and proactive approach to developing your own skills, knowledge and learning Work flexibly between the hours of 7am and 10pm in order to meet the needs of the service. This includes working alternate weekends. Ideally you will have a full driving licence and your own transport as your role will involve visiting service users across the city on a daily basis within a specific time span.
